Verify or Update My Vaccination Status

SPSCC is a fully-vaccinated campus. All students, employees, and contractors are required to be fully vaccinated against COVID-19 or properly exempted from vaccination for medical reasons or sincerely held religious beliefs.

You'll be asked to complete an attestation and then upload a document or image (ie. JPEG, PNG, PDF) of your:

  • CDC COVID-19 Vaccination Record Card; or
  • WA State Certificate of COVID-19 Vaccination; or
  • WA State Immunization Information System Printout.

How to submit your vaccination attestation:

  1. Login with your ctcLinkID and password.

     
  2. Choose Immunization Attestation tile.

     
  3. Complete the form and submit



     
  4. If you have already submitted your vaccination documentation in previous quarters, nothing needs to be done beyond this point. If this is the first time you are attesting, upload your document here: https://apps.spscc.edu/saml/login?destination=covid19vaccination
